YOU WILL NEED TO GO TO THE "UIL WEB SITE" AND PRINT THE Parent and Student Agreement/Acknowledgement Form
Anabolic Steroid Use and Random Steroid Testing.  THIS FORM MOST BE TURNED IN WITH THE  COMPLETED PHYSICAL FORM.
